The resident's application and resident's letter of agreement must be submitted to the Faculty Development Center prior to the beginning of the rural rotation. Please note that the resident's application must be signed by the resident and the residency program director. The letter of agreement must be signed by the residency program director and the rural rotation supervisor.
Upon completion of the Family Medicine Resident's Rural Rotation, the residency program director must submit the Rotation Grant Request Form to the Faculty Development Center. Payment will be made to the residency program upon completion of the rotation evaluations by the director and resident.
